Client for Mobile Physiotherapy

We welcome referrals from general practitioners, support coordinators, case managers, hospital discharge planners, and other allied health professionals.

Here’s how to refer your client for in-home physiotherapy:

Gather Client Details

Please include:

  • Full name and date of birth
  • Contact number and home address
  • Relevant medical history and current concerns
  • Funding type (e.g. CHSP, HCP Level 1–4, NDIS, private)

Attach Relevant Documents

If available, please provide:

  • Recent discharge summaries
  • Medical reports
  • Physiotherapy referrals or care plans
  • Risk assessments or mobility alerts

We Contact the Client

Once the referral is received, we will:

  • Contact the client or family within 1–2 business days
  • Arrange an initial in-home assessment
  • Provide progress updates to the referrer as requested
